Job Description

Community Health Worker (CHW) – Field-Based (CalAIM Program)
Schedule : Monday – Friday | 8:30 AM – 5:00 PM
Work Structure : Approximately 65% field-based / 35% remote
Compensation : $21.00 – $24.00 per hour
FLSA Status : Non-Exempt
Location : Hybrid – Field Based (County-Based Assignment)

The Community Health Worker (CHW) serves as a vital link between healthcare systems and vulnerable populations, supporting members through direct engagement, education, and coordination of services that improve overall health and stability.

What This Role Looks Like (Day-to-Day Reality)

  • Spend approximately 65% of your time in the field (homes, community sites, events)
  • Conduct outreach, assessments, and direct member engagement
  • Travel within assigned county to meet members and community partners
  • Support health education, care navigation, and resource coordination
  • Facilitate group sessions and community-based activities
  • Document services and interactions in real-time
    This role requires strong time management, independence, and comfort working in diverse, community-based environments

What You’ll Do

Direct Member Services & Care Coordination

  • Conduct comprehensive assessments and develop Plans of Care
  • Provide ongoing support and follow-up with members
  • Assist members with care navigation, appointments, and service coordination
  • Support transitions of care and continuity of services

Health Education & Navigation

  • Deliver health education and coaching to improve self-management
  • Assist members in understanding healthcare systems and accessing services
  • Address barriers such as transportation, language, and access to care

Community Outreach & Engagement

  • Conduct outreach to identify and engage high-risk, hard-to-reach populations
  • Participate in community events, wellness fairs, and outreach initiatives
  • Coordinate mobile health clinics, vaccination drives, and community activities
  • Build trust within the community and support program awareness

Group Facilitation & Education

  • Facilitate group sessions and workshops on health-related topics
  • Provide education on prevention, mental health, substance use, and wellness
  • Support community-based learning and engagement initiatives

Advocacy & Support Services

  • Assist members with enrollment in public assistance programs
  • Provide advocacy to improve access to care and services
  • Support members in navigating complex healthcare and social systems

Documentation & Collaboration

  • Maintain accurate, timely documentation of all services and interactions
  • Complete required reporting and billing documentation
  • Collaborate with interdisciplinary teams including RNs and licensed providers
  • Participate in case conferences and team meetings

Requirements

Minimum Qualifications

  • High school diploma or equivalent; Associate’s or Bachelor’s degree preferred
  • Minimum of 2 years of experience in community health, social services, or related field
  • Strong understanding of community resources and social determinants of health
  • Excellent interpersonal, communication, and engagement skills
  • Proficiency in electronic health records and basic computer systems
  • Bilingual skills preferred (Spanish or other relevant languages)
  • Must be proficient in technology, including documentation systems, EHR platforms, and communication tools
  • Ability to document in real-time and manage multiple systems effectively

Work Requirements

  • Ability to travel extensively within assigned county
  • Must reside within assigned service area
  • Ability to work occasional evenings or weekends for community events
  • Ability to work in a hybrid, field-based environment

Additional Requirements

  • Possess a valid California Driver’s License (Class C minimum)
  • Maintain a personal, operable vehicle for daily business use
  • Maintain current liability insurance that meets California’s minimum legal requirements
  • Must pass a Motor Vehicle Report (MVR) background check prior to employment
